following historical abuse inquiries and ongoing systems issues
• Reduced staffing or issues transporting prisoners to hospital appointments
• Education required around healthcare and chronic health conditions
• Missed hospital appointments need rescheduled/flagged
• Mental capacity may need assessed for repeated refusal of healthcare
• Professional conduct needs to improve
• Failure to access GP medical records, or follow-up on blood test results
• GPs not consulting with prisoners before prescribing medication
• Limited access to prescribed medication out of hours
• Poor communication within prison / with external healthcare providers
• Independent support person / liaison needed for bereaved relatives
